Davis Test Kits Return Policy

1) Returns are only accepted within 30days of purchase and only if Davis Test Kits has breatched our agreement with you.

Our commitment or agreement with you is as follows...

a) Our kit(s) will ship within 24hrs. of your order date. Please note that outbound shipping only occurs M-F excluding all government hollidays.

b) Your kit will contain the nessasary materials for you to collect your sample. Each kit is guaranteed to collect one sample per order.

c) Your kit will contain direct contact information to laboratory services as well as customer service.

d) Your kit will contain direction to the collection process and shipping as well as the offer to assit you over the phone or in person (for a fee)

e) All results are due five business days after the receipt of sample (not counting the day received) Note that all, "Chain of Custody" forms must be completed including order number and signature. Any information not completed will void this guarantee of delivered results. This policy applys also for two day and same day expidited results. If the claim is a lost returned kit, you must include tracking information. The customer is responsible for assuring the kit is returned to us not damaged and in tact.

f) Your results will be delivered to you per your request. Results over the phone only guarantee our attmept is timely, if you are not home, a voice message will be left. If you are to receive your results via email, we wil attempt to send with a, "Word" documnet attachment, and if we mail your report, we assure it will be mailed on the date due. Please note that we can not warrant the receipt of your voice mails, emails, ability to open files, or the delivery of your results.

If we have not fullfilled any of the items within a-f, we also require that you have given us the opprotunity to make it right. If we can not make it right, a refund will be issued.

1) ALL fields within this form must be completed electronically and may not be hand written in.

2) This form must be signed for refund consideration.

3) Both printed pages must accompany your returned kit.

Shipping cost are not refundable. By making this refund request and signing this form, you agree to the terms of our agreement to receive only a refund on the cost of the test kit returning and that any shipping cost paid to us to ship the kit to you or any incurred cost shipping the kit back to us is not reimbursable. You further agree that this dispute will be considered closed and will not hold Davis Test Kits, its employees, and or any representative associated with Davis test Kits harmful in any way. I also confirm by signing that I was the person who initiated and completed the order for this kit and am at least 18 years of age and a legal citizen of the United States of America.
